The chronic disease management and prevention sub-aim focuses on health care best practices and support services to improve prevention, early identification and management of chronic conditions. The Midwest QIN-QIO supports prevention and chronic disease management by partnering with providers and outpatient clinical practices to improve outcomes in four priority areas: vaccinations, Type 2 diabetes, hypertension, and chronic kidney disease.

Through the use of CMS-approved, evidence-based strategies and data-driven interventions, the Midwest QIN-QIO helps ensure beneficiaries receive timely and effective care that prevents disease onset, slows disease progression, and reduces adverse events. We emphasize the importance of vaccination education and reporting, routine screenings, and provider technical assistance for conditions such as diabetes and hypertension. Additionally, the Midwest QIN-QIO promotes the use of data analytics, artificial intelligence (AI), and population health tools to monitor progress and drive quality improvement across the care continuum.
